For Anesthesia and Pain services billing questions you may call our Billing Office Monday – Friday from 9:00 a.m. to 6:00 p.m. at 800-222-1442.
Many patient questions we receive relate to the fact that patients at our sites receive two separate bills for their care. One bill relates to a facility charge from the Facility for CRNA services, and the second invoice relates to services provided by our physicians. Should you have questions related to our services, please contact us.

You may conveniently pay a bill on line. You will need the following information to complete the payment transaction:
- Patient’s Name
- Phone Number
- Account Number
- Date of Birth
- Credit Card Type (Visa, Master Card, Discover, American Express)
